Quality of Life Worksheet

This is not a substitute for your veterinarian’s assessment of your pet. However, this worksheet may be useful for you in evaluating quality of life. Please seek out help from a qualified veterinary provider to care for your beloved pet and guide you through difficult decisions when they are ill.

Quality of Life Worksheet
Think of the last time your pet was healthy.
This should be in the last year or so. Don't go all the way back to when your pet was very young.
Consider what was "normal" for your dog or cat as a healthy adult.
You might want to look at photos or videos of your pet to remember what healthy behavior was like.

Answer these questions in comparison to that ideal.

An animal that has trouble breathing, or that is shaking or trembling, may be in distress. PLEASE SEEK OUT ATTENTION FROM A VETERINARIAN. This may turn into an emergency situation.
